Language Assistance

The CDI Group maintains a free language assistance program for those whose preferred language may not be English. CDI will provide a staff member who is fluent in your preferred language or promptly arrange for interpretative services over the telephone. CDI can provide translated documents, including the provider directory, at your request. Membership Agreement

OneSmile Plan is required to provide prospective members a copy of the Individual and Family Combined Membership agreement and Description of Services and Disclosure Form (DOS/DF) prior to enrollment. Please locate your appropriate state above and click on the link to view/print your DOS/DF.

As a member, you pay a low yearly fee. After joining, visit a participating OneSmile Provider – simply show your Member ID card at the time of your dental treatment. You will pay your dental office only for the care you want and need at a discounted rate. It’s that easy!
